Notice of Privacy Practices (HIPAA)

This Notice describes how medical and dental information about you or your child may be used and disclosed and how you can access this information. Please review it carefully.
Our Duties
- Maintain the privacy and security of Protected Health Information (“PHI”).
- Provide this Notice and follow the privacy practices described here.
- Notify you following a breach of unsecured PHI when required by law.
How We May Use and Disclose PHI
- Treatment: Share PHI with dentists, physicians, or other professionals involved in care, including referrals and care coordination.
- Payment: Use/disclose PHI to bill and obtain payment from you, insurers, or third parties (eligibility checks, preauthorizations).
- Health Care Operations: Quality assessment, training, credentialing, audits, and business management.
- As Required by Law: Public health activities, reporting abuse/neglect, health oversight, law enforcement, court orders, and to avert serious threats.
- Individuals Involved in Care: With your permission or when permitted, share limited PHI with a parent/guardian or person involved in care or payment.
- De-identified Data: We may use/disclose de-identified information that cannot reasonably identify an individual.
Uses and Disclosures Requiring Authorization
For certain purposes (e.g., most marketing, sale of PHI), we will obtain your written authorization. You may revoke an authorization at any time in writing, except to the extent we have already relied on it.
Your Rights
- Right to Access: Receive copies of PHI, including electronic format when available.
- Right to Amend: Request corrections to PHI you believe is inaccurate or incomplete.
- Right to Request Restrictions: Ask us to limit use/disclosure of PHI (we will consider requests; we must agree to certain restrictions when you pay out of pocket in full).
- Right to Confidential Communications: Request contact by alternative means/locations.
- Right to an Accounting: Receive a list of certain disclosures made in the past six years.
- Right to a Paper Copy: You may request a paper copy of this Notice at any time.
Special Protections
Some categories of information (e.g., mental health records, substance use treatment records) may receive additional protections under federal or Texas law.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate for filing a complaint.
